The most recent appropriations for the Department of War expired at 11:59 p.m. EST on September 30, 2025. Military personnel will continue in a normal duty status without pay until such time as a continuing resolution or appropriations are passed by Congress and signed into law. Civilian personnel not engaged in excepted activities will be placed in a non-work, non-pay status.

During this partial government shutdown, the 61st Medical Squadron remains open and continues to honor scheduled medical and dental appointments. Our patient appointing service is open and we're still taking requests for appointments. We're doing our best to minimize impact on our beneficiaries and their health care. The clinic’s Primary Care and Specialty Care services are fully operational. Ancillary services (pharmacy, laboratory, radiology) also remain operational. Disease Management  

Disease Management empowers eligible 61st Medical Squadron beneficiaries to become active partners in their health care to achieve improved health outcomes. 

Who We Serve 

The 61st MDS DM program serves empaneled patients, ages 18-64, with the following chronic health conditions: 

  • Asthma 
  • Diabetes Mellitus type II 
  • High cholesterol 
  • Hypertension (high blood pressure) 

What to Expect from the 61st MDS Disease Management Program 

The DM specialist will assess patient needs and educate them about essential skills needed to help them to better manage chronic conditions, with a heavy focus on the following:  

  • Education and empowerment to better monitor conditions and promote healthier lifestyles  
  • Identify roadblocks and obstacles to treatment 
  • Improve understanding of chronic health conditions 
  • Assist patients in medication management and compliance 
  • Expectations for treatment 
  • Promoting effective communication with the patient’s primary care team 
  • Provide patients with educational resources and recommendations 
  • Assist patients with specialty referrals
