For all other Public Health services, please walk-in Monday through Friday, 7 a.m. to 4 p.m. PHAs are no longer conducted by Public Health, they are completed by BOMC. BOMC can be contacted at 310-653-6850. A web-based PHAQ is an annual requirement for all Air Force personnel, and must be completed within 10 days of ASIMS notification. Airmen who do not complete the AF PHAQ within the specified time period shall receive a follow-up notification from their Unit CC, First Sergeant, or UHM. Failure to comply will have a negative impact on your individual medical readiness and ability to deploy. Use this link to view your current Individual Medical Readiness status at MyIMR.
